Health Homes

Clients can benefit from our Health Homes programs if they are on a Medicaid plan and have two or more chronic conditions, have one chronic condition and are at risk for a second, have one serious and persistent mental health condition. The purpose of this program is to help reduce visits to the emergency room and time spent in the hospital while increasing their access to their routine primary care. Our Care team provides high quality prevention and health screenings to clients while supporting their medical needs.

Eligibility information

Eligible clients must have a Medicaid insurance plan in order to take advantage of Health Homes services. If an individual requires a high utilization of emergency medical services, they may benefit from services provided by this program.

Diagnosed with two chronic conditions
Diagnosed with a chronic condition and at-risk for a second
Diagnosed with a serious mental illness
