Health Homes

Clients can benefit from our Health Homes programs if they are on a Medicaid plan and have two or more chronic conditions, have one chronic condition and are at risk for a second, have one serious and persistent mental health condition. The purpose of this program is to help reduce visits to the emergency room and time spent in the hospital while increasing their access to their routine primary care. Our Care team provides high quality prevention and health screenings to clients while supporting their medical needs.

Assertive Community Treatment (ACT)

This specialized community-led care team is dedicated to serving individuals ages 18+ who have more severe and persistent mental health diagnoses (including Bipolar Disorder, Schizoaffective Disorder, Chronic Major Depression and Schizophrenia) in order to help improve functionality.
