This specialized community-led care team is dedicated to serving individuals ages 18+ who have more severe and persistent mental health diagnoses (including Bipolar Disorder, Schizoaffective Disorder, Chronic Major Depression and Schizophrenia) in order to help improve functionality.

Eligibility information
In order to qualify for specific services from our ACT Team, clients must be at least 18 years old and diagnosed with a serious mental illness. Many clients who join this program have been unable to meet their needs through traditional treatment plans and outpatient services.
Inability to participate in traditional outpatient treatment
Two or more psychiatric inpatient admissions over 12-month period
Excessive use of emergency services
Persistent severe symptoms
Unhoused or at risk of being unhoused
Repeated arrests or incarceration
Co-existing severe substance use disorder
