Single adults or adult families who are homeless and living with a severe mental illness and/or substance use disorder are eligible to apply. This housing program only accepts referrals through HRA’s PACT System with a 2010e housing application form.

Eligibility information

The program provides permanent supportive housing units to single adult or adult families who are homeless and living with a severe mental illness and/ or substance use disorder. We also serve young adult age 18-24 who were either in foster care or have been homeless and are living with at least one child under the age of 18. This particular housing program only accepts referrals through HRA’s PACT System with a 2010e housing application form.
