Supportive Housing

BAC is committed to securing our community with supportive housing.

Available programs

Our team prioritizes providing shelter to individuals with chronic medical conditions, mental health conditions, a history of substance use, young parents, as well as those who have been homeless or formerly incarcerated. We have connected formerly homeless individuals and families with more than 260 open units through extensive programs.

OASAS MRT Scatter-Site Housing Program

Similarly to Health Homes MRT, this OASAS program provides scattered-site apartments to high users of Medicaid who are homeless, however this program is specifically for those living with a qualifying substance use disorder. Clients are only accepted through referrals from HRA’s PACT System with a 2010e housing application form.

OASAS NYNY III Category F Scatter-Site Housing Program

This dedicated program has similar qualifications as OASAS MRT, however, services are strictly for clients with a history of substance misuse disorder who have completed substance use treatment.

DOHMH NYC 15/15 Scatter-site Housing Program

Single adults or adult families who are homeless and living with a severe mental illness and/or substance use disorder are eligible to apply. This housing program only accepts referrals through HRA’s PACT System with a 2010e housing application form.

Young Adult Families (YAF)

Our Young Adult Families program is for heads of the household who are pregnant and between the ages of 18-24 seeking housing. We help improve an individuals’ quality of life with stable housing that can help to decrease high service utilization.
